Promoting people’s well-being from a multifaceted perspective
Promoting the holistic well-being of older adults through a 360º approach
POBA emerges as an innovative socio-healthcare model that seeks to respond to the health and social needs of older people through a single system that brings together and coordinates healthcare and social services. Its ultimate goal is to improve the overall well-being of older adults.
This project, promoted by ADINBERRI, stems from two experimental projects supported by the Foundation: the project “Development of the governance of the Aretxabaleta socio-healthcare center” (promoted through ADINBERRI’s 2022 funding call), and the project “AGASE Ecosystem Model” (promoted through ADINBERRI’s 2019 funding call). In addition, POBA has drawn on the Zaintza Herrilab project of ADINBERRI.


In line with the New Model of Care and Support in Gipuzkoa
POBA represents a paradigm shift in the care and support system, moving from a mainly reactive model to one that is preventive, proactive, personalized, and coordinated, in which the different systems—healthcare and social—act jointly and complementarily.
The person takes a central role in the intervention, not only as a recipient of services but as an active subject of rights and care.
Promoting functional capacity to prevent frailty
In the face of increasing life expectancy, the challenge lies in fostering the maintenance of functional capacity by acting on modifiable factors—such as frailty—that are therefore subject to intervention. Frailty is a state of pre-disability and pre-dependency that can be prevented, identified, and reversed, making proper management of it critically important.
POBA is firmly committed to the active prevention of frailty through the early identification of risks such as falls, polypharmacy, cognitive decline, or situations of loneliness, and through the development of personalized programs aimed at maintaining autonomy and delaying dependency.

With the aim of testing the POBA model, the Basque Government’s Department of Health, the Provincial Council of Gipuzkoa—through the ADINBERRI Foundation—and the Aretxabaleta City Council have launched a pilot project in Aretxabaleta. This initiative integrates healthcare, social, and community services in a single space, focusing on prevention, coordination, and personalized care for people over 70 years of age.
Osakidetza has identified that 1,272 people over the age of 70 live in aretxabaleta, of whom:
